AN Military officer was crushed to demise by an armoured personnel service after a safety alarm had gone unfixed for years.

Second Lieutenant Max George, 26, acquired out of the Warrior moments before its driver "reversed at velocity" and unknowingly ran him over.

The crew have been 40 metres away before they realised he had been hit through the exercise on Salisbury Plain, Wilts, in 2022.

The alarm would have alerted the driving force that the car's rear door had been opened and would have "virtually definitely" saved Lt George's life.

It broke in 2018.

An investigation found the Army was "over-committed and under-resourced" which put troopers at "considerable danger".

Air Marshal Steve Shell, of the Defence Security Authority added: "This accident was preventable and there are areas we must improve upon immediately to stop it occurring again."

Lt Col Jim Hadfield stated of Lt George: "He was a pure commander who exuded fierce willpower matched by selfless compassion."

The Army stated: "We have now accepted all the suggestions, together with further safety measures for the Warrior rear exit."
